SACAA: ZU-AZP Accident report outstanding 2009
Dear SACAA,
I had an accident reported to you pertaining an engine-out at FABS on 10 January 2009 on aircraft ZU-AZP.
Numerous calls, enquiries and beggings on the outcome of this accident have been ignored by you. Thus my fall-back on a public forum.
You have my call-sign, you have my report, therefor you know where to reach me.

Not every "accident" is investigated to the extent that a report is written. The purpose of accident investigation is to determine a root cause so that a re-occurence can be prevented. I am advised that in this case, it was evident that the engine had not been operated and maintained according to manufacturer's specifications by the owner, resulting in the engine failure. Minimal damage to the aircraft was sustained and the Accident and Incident Investigation Division decided only to record the incident on the database.

Is there any way that I, as the ownwer, can be informed by the investigator or CAA of the reasons for his/her findings ?
Surely such an allegation is important for the owner to know, yet I have never been informed by the investigator of his/her shocking findings.
It is important for my own survival (and thousands of Rotax owners out there) to learn where we incorrectly "operate and maintain the engine according to manufacturer's specs", because it sounds that all of us have been taught the incorrect way. All this notwithstanding 8 years of AP's and a full service history in the airframe logbook ?
